Concerro is a web-based staffing software and workforce management solution for health care professionals. By reducing labor costs through optimization rather than strict reduction, it helps to improve both the quality of care and labor costs.

The ConcerroConnect online community is designed to bring together nurses and health care management staff to explore innovative ideas, collaborate with colleagues and lead an open dialogue on issues in health care reform. Quality Improvement &
Innovation Partnership

The Quality Improvement & Innovation Partnership (QIIP) is a provincial organization funded by the Ministry of Health and Long-Term Care. QIIP strives to introduce, integrate and spread quality improvement methods and practices in the provincial health care system.

In order to build a learning community among primary healthcare practices, QIIP created a virtual place for primary healthcare teams to learn, share and innovate with one another in order to improve patient care. The online community provides a knowledge exchange for quality improvement efforts in chronic disease management and prevention, office practice redesign and technology innovation.

Pipeline Healthcare develops and delivers world-class telepharmacy, leadership development and online community services for hospitals and retail pharmacies. PipelineRx, their proprietary telepharmacy platform, provides access to a team of licensed pharmacists 24 hours a day, 365 days a year.

Pipeline built a unique community specifically for pharmacy directors, a niche subset of the medical community that lacks adequate representation online. Pharmacy directors and their colleagues now have a place to network with their peers and find up-to-date news, interesting articles and useful forms. They are also given information about continuing education and business best practices.

TransforMED

TransforMED, a wholly-owned subsidiary of the American Academy of Family Physicians (AAFP), provides ongoing consultation and support to physicians looking to transform their practices to a new model of care that is based on the concept of a patient-centered medical home (PCMH). Facilitators use lessons learned from projects to support primary care practices across the country as they implement PCMH within their practice.

TransforMED created the Delta-Exchange network for primary care practice transformation. The online learning community provides useful tools and best practices to help primary care practices transform to patient-centered medical homes via an easy-to-use, private, collaborative website where practices can ask questions, share knowledge and discuss their experiences.
